What is the difference between endometriosis and adenomyosis?
In endometriosis, tissue similar to the uterine lining grows outside the uterus (on ovaries, fallopian tubes, or pelvic peritoneum). In adenomyosis, this endometrial tissue grows deep into the muscular wall of the uterus itself, causing an enlarged, tender uterus and painful heavy periods.
What are the classic symptoms of endometriosis?
Common symptoms include severe menstrual cramps (dysmenorrhoea) that worsen over time, chronic pelvic pain, pain during or after intercourse, painful bowel movements during periods, and difficulty conceiving.
How is endometriosis definitively diagnosed?
High-resolution pelvic ultrasound and MRI can identify ovarian endometriomas (chocolate cysts) and deep infiltrating lesions. Diagnostic laparoscopy is the gold-standard surgical method to definitively visualize, stage, and treat endometriosis implants.
Can laparoscopic surgery improve fertility in women with endometriosis?
Yes. Laparoscopic excision of endometriotic implants, cystectomy for ovarian cysts, and mobilization of pelvic adhesions restores normal pelvic anatomy, significantly improving spontaneous pregnancy rates and IVF success.
What medical therapies are used to manage endometriosis symptoms?
Medical treatments include hormonal oral contraceptives, progestin-only pills, dienogest, hormonal IUDs (Mirena), and GnRH analogues to suppress oestrogen production and prevent active lesion bleeding.
